Endorsement of Children on parent's passport or birth registration you have to provide the following:

1. Duly filled in application form called form-C for Endorsement on Passport
2. Duly filled in application form called form-S for Birth Registration
3. Full birth certificate in original along with its photocopy
4. Mother's passport in original (in case of endorsement on passport)
5. Photo copy of Father's passport (First 4 pages only)
6. No Objection Certificate from non Pakistani Mother (attested by notary public) if a child is to be endorsed on father's passport
7. The fee should be remitted by payment slip or deposit fee in Embassy `s account 235-412500.01H UBS Bern. Please attach a proof of payment.
8. Self addressed and Stamped envelop
9. CNIC's / Swiss Residence Permit copies
10. The fee: Registration: CHF 5.00, Endorsement Fee: CHF 7.00,
    (Urgent service Fees are doubled)
11. Kindly remit fee by postal payment slip or bank deposit in Embassy's account 235-412500.01H at UBS AG, 3000 Bern. Please attach copy of the payment.(cash is not accepted)
